STEP 1- SUBMIT AN
ON-LINE GRADUATE SCHOOL APPLICATION AND PAY APPLICATION FEE
1. See the
Graduate School application for
details on this step.

The
Graduate School application contains detailed instructions on how
to apply. See the Graduate School checklist for information on
which materials to send to the Graduate School. You must apply
electronically through the Graduate School's Web site. Please send
the application
fee to the Graduate School at the address on
their website (use link provided above). |
2.
Contact Educational Testing Services (ETS) to arrange for official
GRE (and the TOEFL
if required) test results to be sent to the
Graduate
School. Use institution code 1846. A department code is
not needed.

3. Submit
three letters of recommendation from individuals
who can assess your potential for success in graduate study.
At least one letter must be from a college or university faculty
member with whom you have studied. These may be submitted online
or submited as a hardcopy to the department in a sealed enveloped.

STEP
2- SEND REQUIRED MATERIALS TO THE DEPARTMENT OF POPULATION
HEALTH SCIENCES
1. A
statement of purpose. This statement may be
submitted with your online graduate school application, or
you may choose to submit it directly
to the department. The statement should be no more than 2
pages in length and address the following topics: |

a) Your reasons
for wanting to enroll in the University of Wisconsin-Madison Population
Health Graduate Program.
b) Your educational goals as a graduate student.
c) Your research interests and career objectives.
d) Your proposed
area of concentration (epidemiology, health
services research, or clinical research). |
| 2. Two copies of official
transcripts from each school attended. Documents must
be issued by the school with the official seal/stamp and an official
signature. International academic records must be in the original
language accompanied by an official English translation. |
| 3.
Include one copy of a current resume or vitae. |
| 4. Mail
all materials, preferably in one large envelope, to:
